Steel Demon spawned in Ironwing Lair, Olandra, lvl 108 on easy mode, at 307 points. Location was right around the corner from boss, easy to find!

Lava spawned in Barlem's Lair, Alentia, lvl 108 on easy mode, at 307 points. Location is hidden, off main path, to the right, other side of water/river. ( i didn't even know about this part of the dungeon) (Opposite of the path that goes left to where Crystal Tooth spawns)

Black Smog spawned in Deyarka Tunnel as lvl 108 for me on easy mode (lvl 50 mobs).

i was doing the dungeon by myself on easy mode. i'm lvl 108 and the mobs are about lvl 48.

Warning! Burnt Shadows is lvl 1 in Well of Prophecy dungeon, Wynnadia, on easy mode. Took me over 3 months to get 323 points doing it on easy mode, and it was only lvl 1. And even though i took off my weapon, and put my dragon away, i accidentally killed it with 1 swipe of noob (training)...
